Required enrollment Forms

To help us get ready to welcome your child, we’ll need a few admittance forms completed:
  • Emergency Contact Form – This is a fillable PDF. You may print and fill it out by hand, or type directly into the form and email it to preschool@westlawnumc.org. Click the button below to access this form. 
  • Child Health Report & Immunization Records – These must be completed and signed by your child’s doctor. Click the button below to access this form.
  • Enrollment Agreement & Permissions - Completed and submitted electronically (below).

Copies of these forms will also be mailed to registered families in July upon request, so you’ll have plenty of time to complete them.

Medical Authorization & Liability Waiver 

I am the parent/guardian of my child previously listed and agree to the following:

I understand and agree that my child will participate in the daily activities of West Lawn UMC Preschool (“the Preschool”), which may include occasional trips to the West Lawn Library and Playground. These outings involve leaving the Education Building and crossing a public street under staff supervision.

The Preschool rooms are prepared with age-appropriate safety measures, and all children are closely and consistently supervised; however, we recognize that accidents may occur. I release West Lawn UMC Preschool (including its staff, volunteers, directors, and leaders) from liability in connection with my child’s attendance and participation in all activities, including but not limited to classroom time, playground use, and field trips. This release applies for the full duration of my child’s enrollment.

I authorize Preschool staff to obtain medical care for my child if, in their judgment, it is needed. If my child is released to me, my designee, emergency personnel, or a medical facility, the Preschool is released from further responsibility. I accept financial responsibility for any related costs of medical care or transportation and agree to hold the Preschool harmless from any related claims.

In the event of an emergency, staff will follow the Emergency Contact Form provided at enrollment.

Receipt & Understanding of Preschool Handbook

I acknowledge that I have received and read the West Lawn UMC Preschool Handbook, which is available at any time on our Parent Resource webpage.
  • Tuition Payments
    I understand that I am financially responsible for making tuition payments on time, according to the tuition schedule provided in the handbook.
  • Timely Pick-Up
    I understand that I am responsible for picking up my child on time. After the first verbal warning, any late pick-up will result in a $10 charge for every 15 minutes.
  • Discipline Policy
    I have read the discipline policy and understand that I will be informed of any behavior concerns. I agree to work cooperatively with the teacher to help my child improve their behavior.
  • Parental Code of Conduct
    I have read and understand the section of the handbook regarding parental code of conduct. I acknowledge that my conduct may affect my child’s enrollment in the program.

Monthly Tuition Schedule

Monthly tuition is due by the 10th of each month for the following month.
A $10 late fee will be applied to payments received after the due date.
Tuition is due regardless of sick days, snow days, or absences.
